NSDRC STAGE Family Survey (June 2020)

STAGE is in the process of planning for resuming face-to-face services. This service will only be offered to up to 3 participants for each block of time.

Our plan is for one employee to offer a morning class and an afternoon class.  Employee will be sanitizing the high touch areas and common spaces between classes and we will not be able to have participants in the building between classes.  Complete plan will be made available prior to returning to STAGE.

1. I am interested in having my participant return part-time to STAGE. If NO is checked go to question #5 *
2. Classes will be offered between 10am to 12pm in the morning. My participant will have transportation to attend STAGE and leave promptly at the end of class.
3. Classes will be offered between 1:30pm to 3:30pm in the afternoon. My participant will have transportation to attend STAGE and leave promptly at the end of class.
